Certain equivalencies or documented clinical experience may be considered under the official rules.

Applicants should therefore review the nature of their actual professional responsibilities rather than relying only on the total number of employment years.

5. Are Examinations and Interviews Required?

Yes.

The PRO application process involves more than submitting a medical degree and employment certificates.

It is organized into three principal stages.

Phase 1: Eligibility Review

Touchstone Institute reviews immigration status, medical education, language proficiency, licensing qualifications, postgraduate training, independent practice experience, required clinical rotations, and recent clinical practice hours.

This stage determines whether the applicant meets the preliminary eligibility requirements.

Phase 2: Verification and Selection

Eligible applicants submit supporting documentation and complete the required examination and interview procedures.

These may include the Therapeutics Decision-Making Examination (TDM) and applicable Medical Council of Canada examination requirements.

Applicants may also be required to pursue provisional registration with the CPSO.

Phase 3: Clinical Field Assessment

Selected physicians complete a 12-week supervised clinical assessment in an approved Ontario setting.

Before starting, they must enter into a three-year Return of Service agreement with the Ontario Ministry of Health and obtain the necessary provisional CPSO registration.

The program is competitive.

Meeting minimum eligibility requirements does not guarantee selection for an assessment position.

6. Does the 12-Week Assessment Lead Immediately to an Unrestricted Licence?

Not automatically.

One of the main advantages of PRO is that eligible experienced physicians may demonstrate readiness to practise without repeating a conventional full Canadian residency pathway.

However, the 12-week assessment should not be confused with the immediate award of an unrestricted licence.

Physicians who successfully complete the assessment may practise under the conditions established by the CPSO and the Return of Service program.

Supervision and other restrictions may apply during the subsequent service period.

The physician must later satisfy the applicable requirements for an independent practice licence without the relevant restrictions.

Consequently, there is an important distinction between obtaining authorization to begin medical practice under the PRO pathway and obtaining an unrestricted licence for independent practice.

Important: Describing PRO as a program where international doctors become fully licensed in just 12 weeks is misleading. The full pathway includes prior eligibility assessment, examinations, clinical evaluation, and a three-year service commitment.

7. What Is the Three-Year Return of Service Requirement?

Successful PRO candidates must commit to providing full-time family medicine services in an eligible Ontario community for a minimum of three years.

This is known as Return of Service (ROS or RSO).

The obligation generally begins after the candidate successfully completes the clinical assessment and proceeds into the approved practice arrangement.

The program prioritizes communities experiencing shortages of family physicians.

Eligible communities may include:

  • Rural Ontario communities with a Rurality Index of Ontario (RIO) score of 40 or higher.
  • Northern Urban Referral Centres, including Timmins, North Bay, Sudbury, Sault Ste. Marie, and Thunder Bay.
  • Designated communities with high levels of unmet primary care needs.

These locations reflect the program's purpose of improving healthcare access in underserved parts of Ontario.

Candidates should not assume that they can freely select any preferred location, including central Toronto, for their three-year service period.

The relevant employment arrangement, approved location, and service obligations must be reviewed before participating.

8. Is There a Separate Opportunity for French-Speaking Physicians?

The October 7 announcement includes plans for a dedicated stream for French-speaking internationally trained family physicians.

The objective is to improve access to healthcare in Ontario's Francophone communities as well as other underserved regions.

However, French proficiency does not automatically waive the underlying medical education, professional experience, examination, or immigration status requirements.

The program's operating organization should be consulted for the detailed eligibility criteria and implementation requirements applicable to the French-speaking stream.

9. Medical Licensing and Permanent Residence Are Different Matters

From an immigration perspective, this distinction is essential.

PRO is a professional medical assessment and licensing pathway, not a new Canadian permanent residence immigration program.

At present, the program requires applicants to be Canadian citizens or permanent residents.

It would therefore be incorrect for an internationally trained physician to assume that applying to PRO automatically creates a pathway to Canadian permanent residence.

Equally, a physician who already holds Canadian permanent residence must still meet the medical training, examination, and professional requirements of PRO.

The two processes serve different purposes.

Physicians should therefore evaluate their immigration status separately from their professional licensing qualifications.

10. How Does This Relate to Canada's Physician Immigration Initiatives?

The federal government has also introduced immigration measures intended to attract and retain qualified physicians.

In December 2025, IRCC announced several targeted initiatives to address physician shortages.

These included:

  • A dedicated Express Entry selection category for physicians with qualifying Canadian work experience.
  • An additional 5,000 federal admission spaces for physicians nominated by provinces and territories.
  • Expedited work permit processing measures for qualifying provincially nominated physicians.

These initiatives are intended to help Canada recruit and retain healthcare professionals.

However, they do not create identical eligibility rules for all physicians trained abroad.

For example, the dedicated Express Entry category for physicians with Canadian work experience requires qualifying Canadian experience. Overseas medical experience alone does not satisfy that particular Canadian experience requirement.

Furthermore, provincial nomination, work permit eligibility, and entry into PRO are separate legal and administrative processes.

The applicable pathway must be assessed according to the physician's actual circumstances.

11. Can Physicians Trained in South Korea Apply?

A physician who graduated from medical school and obtained a medical licence in South Korea may review their eligibility for PRO.

However, holding a Korean medical licence alone does not establish eligibility.

Important factors include:

  • Whether the physician has Canadian permanent residence or citizenship.
  • Whether their medical school and qualification are accepted.
  • Whether they have sufficient family medicine or general practice training and independent clinical experience.
  • Whether they satisfy the recent clinical hours requirement.
  • Whether the necessary language and licensing examinations have been completed.
  • Whether the required clinical rotations are recognized.
  • Whether they are willing to work in an approved Ontario community for three years.

Experience limited to a particular medical specialty may not satisfy the same requirements as the broad practice experience expected from a general or family physician.

Consequently, applicants should assess their specific clinical responsibilities, postgraduate training, and independent practice history against the official criteria.

12. Common Misunderstandings About the Expansion

There are several important points that prospective applicants should understand.

First, the CAD 20 million announcement does not mean that every internationally trained physician will receive an individual scholarship or licensing grant. The funding supports the expansion of the program and its assessment capacity.

Second, the government is not announcing immediate selection of 200 physicians. The objective is to support 50 additional positions annually over four years.

Third, the 12-week Clinical Field Assessment is not the same as the complete application-to-licensing timeline.

Fourth, Canadian citizenship or permanent resident status is currently a fundamental requirement for applicants entering PRO.

Fifth, successful assessment does not necessarily lead immediately to unrestricted independent practice.

Practical takeaway: Internationally trained physicians must distinguish immigration status, professional licensing eligibility, assessment requirements, and actual employment conditions. These issues should be evaluated separately.

13. Immigration Consultation for Healthcare Professionals at Yujin Visa & Immigration

Canada is implementing multiple measures to respond to shortages of healthcare professionals and to improve recognition of internationally acquired qualifications.

However, medical licensing and immigration authorization remain separate processes.

A physician who needs Canadian permanent residence must first identify an appropriate immigration pathway.

A physician who already holds permanent residence must still independently satisfy the professional licensing requirements applicable to their intended occupation and province.
