Medical Exam
Work Permit

Full English Version
My PGWP Restricts Health-Care or Child-Care Work. Can I Remove the Restriction After a Medical Exam?
A Post-Graduation Work Permit is an open work permit, but “open” does not mean that every occupation is automatically permitted regardless of the conditions printed on the document.
Graduates who did not complete a required Immigration Medical Exam (IME) may receive a PGWP containing restrictions that prevent work in certain occupations involving close contact with people. This can become a serious issue when a graduate later receives an offer from a hospital, long-term care facility, school, child-care centre or home-care employer.
Which occupations may be affected?
IRCC identifies examples that include workers in health-care settings, clinical laboratory workers, patient attendants in nursing and geriatric homes, medical students, medical electives and physicians on short-term locums, workers in primary or secondary school settings, child-care workers, domestics, in-home caregivers for children, older persons or persons with disabilities, day nursery employees and similar occupations.
The job title alone should not be the only consideration. Review the actual duties, the work environment and, most importantly, the conditions printed on the current work permit.
A PGWP condition can be changed after issuance
IRCC's current PGWP instructions recognize that a holder may need to change the conditions on a PGWP after providing an additional document such as a medical exam.
This is not an application to obtain a second PGWP. It is a request to change the conditions attached to the existing work authorization. IRCC currently instructs applicants who need to extend or change PGWP conditions to apply on paper.
Official guidance: Post-graduation work permit – How to apply
Completing the medical exam does not instantly erase the restriction
This distinction is critical. Completing an IME with a Panel Physician does not, by itself, rewrite the conditions printed on an existing PGWP that same day.
If a change-of-conditions application is required, the holder must follow the applicable IRCC process and should not assume that a restricted job can begin merely because the medical examination has been completed.
This matters when an employer has set a firm start date. The employee should determine what the current permit authorizes and when the restricted work can legally begin rather than treating the medical appointment date as the authorization date.
Use an IRCC Panel Physician
An immigration medical examination is different from an employer's occupational health screening. A hospital may ask for vaccination records, tuberculosis screening or other employment-related medical documentation, but those requirements do not automatically satisfy IRCC's immigration medical requirement.
An IME must be completed through an IRCC-authorized Panel Physician under the applicable process.
Depending on the application and circumstances, IRCC guidance may allow an upfront medical examination or may provide medical instructions after an application is submitted. Where a graduate already knows that the intended occupation will require an IME, addressing the issue before the initial PGWP application may help avoid a later occupational restriction.
Four points to verify before starting the job
First, read the exact conditions and remarks on the current PGWP.
Second, obtain a detailed job description rather than relying only on the position title. A role called “technician” or “research assistant” may involve very different levels of contact depending on the workplace and duties.
Third, determine whether there is a valid prior immigration medical result or whether a new IME is required.
Fourth, if a change of conditions is necessary, plan the employment start date so that the worker does not perform work prohibited by the existing permit while the change is being processed.
Common mistakes
One common mistake is assuming that an open work permit means there can be no occupational restrictions. Open work authorization generally concerns flexibility regarding the employer, but the document can still contain conditions restricting certain work.
Another mistake is confusing an employer medical assessment with an immigration medical examination. Completing all hospital onboarding health requirements does not automatically remove an immigration condition.
A third mistake is starting the restricted work first and planning to correct the permit later. Work-permit conditions must be respected. Canadian work history and status may later be reviewed in other immigration applications, so a short-term onboarding deadline should not be treated as more important than compliance with the permit.
Agricultural work may involve separate medical-exam considerations
IRCC also identifies medical-exam requirements that can apply to agricultural work depending on recent residence or travel in designated countries or territories. A person should therefore not assume that medical requirements are relevant only to health care or child care.
Documents to organize
Keep a complete copy of the current PGWP, passport, any prior IME documentation, the employment offer, detailed job description, work location information and proposed start date.
A useful review sequence is: permit restriction → actual job duties → IME requirement → need for a change-of-conditions application → lawful work start date.

Practical conclusion
A PGWP may contain restrictions on health-care, child-care, school, caregiving and similar work even though it is an open work permit. In appropriate cases, the holder can complete the required immigration medical examination and apply to change the permit conditions. The medical exam itself, however, should not be treated as immediate authorization to disregard the existing restriction.
